Hartford Change AGEnts Action Awards

Health Home Connect

  • Health Home Connect was developed by the Los Angeles Downtown Women's Center to identify homeless, older adult women and enroll them in Medicaid by linking the Affordable Care Act (ACA) and the US Department of Housing and Urban Development (HUD)’s Coordinated Entry System (CES) housing system.

Honoring the Care Wishes of Nursing Home Residents

  • This project helps social workers and nurses work together to enhance how a facility identifies, documents, and addresses medical care preferences of residents in an emergency situation through the Respecting Choices model. Staff are trained to engage in advance care planning conversations with residents and family and to make system changes in support of person-centered care.

Change AGEnts Networks

Patient-Centered Medical Home Network

  • The Patient-Centered Medical Home (PCMH) Network aims to support better policy and implementation by reframing the PCMH model to establish the value of caring for older adults and defining a set of PCMH models and policy changes that increase the likelihood of better outcomes for older adults.   • The Network has enlisted five geriatric experts in the field of health care including Molly Mettler, MSW, Senior Vice President of Healthwise in Boise, ID.

Dementia Caregiving Network

  • The Dementia Caregiving Network (DCN) works to achieve improvements in services, supports, and care for persons with dementia and their caregivers. Read a summary of the DCN here.
  • Three social workers are included in the DCN’s interdisciplinary team of dementia caregiving experts: co-chair, Nancy Wilson, MA, LMSW, Baylor College of Medicine and the Houston VA Center of Excellence in Health Services Research; and team members, Lisa Gwyther, MSW, LCSW, Duke University School of Medicine; and Katie Maslow, MSW, Institute of Medicine.

Change AGEnts Policy Institute

  • The Change AGEnts Policy Institute provides an opportunity for Change AGEnts to gain knowledge and acquire skills necessary to mobilize for action on policy issues that bring about improvements in healthcare and quality of life for older adults. Institute topics range from federal policy and regulatory change to state and local policy work. The Institute helps AGEnts gain a stronger understanding of the complex policy-making environment and the role that they can play in driving policy outcomes.
